I say this is a case of "Confirmation Bias"... you only looked where you believed it should have been based on past experience...
I had an episode a couple days ago where I was confused between 2HI and 4LO on the transfer case, and making comments about driving on 2HI when I was thinking about 4LO.
Speaking of this ... I have made this type of mistake all my life. 10 years ago, we had a cure process that had issues.. I was asked to help troubleshoot. I told my management that I observed it running at 14 PSIA based on the gauge on it when it was running 1.4 bar (I was used to PSIA so I assumed it was that). I told my management of my blunder a day later and called it "confirmation bias". The plant manager told me... "No... that's just stupidity".. But I sort of disagreed with him internally...
